This contract is entered into between the New Jersey State Board of Medical Examiners (hereinafter referred to as “the Board”) and the healthcare professional (hereinafter referred to as “the Professional”). The Professional acknowledges and agrees to abide by the following CME requirements as prescribed by the Board:
